In the depths on the Undertale: Insanity, sans got crazy all the time, and it's find a way to calm him down, using his green syringe, he still sane and passive but, he's having trauma, and he's not okay, the underworld and hopes and dreams are ruined (just like @chef_pp_daughter reference lol), he's hiding his emotions, he is now a failured unsanity.
