After his brother Papyrus came down with an unknown illness Sans had to step up to help protect him and find a cure. Sans has stopped being lazy and he joined the royal guard. He wears his brother's scarf after Papyrus gave it to him, telling him it was his lucky charm.
In Vipertale there is an outpost on the surface to make sure no humans fall into the underground, there has been no humans in the underground since the barrier was created. This means Asgore, Toriel and Asriel are still a family since Chara never had went to the underground.
Eventually they learn that the virus infecting Papyrus was changing his soul. What they don't know is that he is infected with the fresh virus, but Papyrus has been fighting it so well that it hasn't been able to take over.
