Why is it called Therapy? Its an odd name

The mac file does not have the game in it as an app. It just has the contents and I can't play it because of that.
it looks like a really good game but as i see in the other comments i need different kind of mouse :/ .... god, haha i have a laptop and not the portable mouse :(
