It's a fight between two immortal (Herbert's blood basically prevents him from dying in the comics, and 049 was granted immortality by the Brothers Death) "doctors" who toy with life and death. Also, syringes.
I like how some fanon depictions of post-scooped Michael depic him with a full set of hair as if canonically he isn't a bald fuck post-scooping
