(Part 3)Because Chara erased Frisk, the universe became corrupted since Frisk was an essential element, creating anomalies. One of these anomalies appears before Sans and merges with him.

ANOM!SANS

(Part 4)(it's Frisk, but not completely; it's more like Frisk's determination transformed into an anomaly, retaining some memories and personality). Then Sans fully recovers and goes to Asgore's throne where Chara kills him.
