Sooo, to get this achievement, you need to get to shift 15 without any coffee or chocolate. Someone said the mint tea doesn’t count, but I’m not 100% sure, so I didn’t use it.
HERE’S WHAT TO DO:
First, credit and thanks to the person who gave this tip online, use the Secretary skin and buy the max tasers to tase anomalies.
What I did:
Equip/buy Secretary skin (it’s 120 animal coins).
Speedrunning is your best friend! I left fires in rooms (at some points) so I could then throw the fainted patients in the trash, if they don’t faint they are an anomaly so tase them (+2 sanity). HOWEVER that apparently is one of the things that triggers the Ghost to spawn, The Ghost didn’t appear for me but if you’re worried about it then don’t throw the patients away (as it’s a super annoying anomaly to deal with especially to get this achievement). Also use the rechargeable taser when you can so you don’t waste your bought one when you don’t have to. ANOTHER THING TO BUY is the second check in window so you can get through that bit faster. Let in anomalies (each check in is +1 sanity) and tase them right after you check them in (+2 sanity AND -1 patient to treat!). As for the rest of the patients, treat them as quickly as you can manage so your sanity doesn’t slowly lower. On nights when the ambulance comes, treat the patients like normal, if an anomaly appears let them attack a patient (so that you don’t have to treat the patient after as they will run away) before tasing the anomaly.
YOUR ENEMIES ARE: Stalker (he appeared like three times during my run bro wanted me to fail I swear), Bed Monster (easily avoidable or give it maple syrup), Hiders (check the walls of the rooms when you enter, you can spray them with the fire extinguisher or just run away), Eyeball monster (easy to avoid just don’t look up) and anomalies you let in (if you accidentally let one in, either tase them, let them attack one of the patients OR you can lead them to one of the patients that are waiting for an appointment by the hospital entrance)
When the slime shift appears, (for some reason Slimebert didn’t come when I was playing) when Slimebert appears, avoid walking through the hospital as he will be roaming and he will take your sanity if you run into him (you have to wait until he leaves the next night). Again, speedrun the patients if you can. I unfortunately don’t really have tips on slime shift as I only went through them twice and again, Slimebert didn’t spawn one time.
