Originally, completely filling the Toxic meter wouldn't kill you, it just forcibly took your mask off and you wouldn't be able to use it again until the meter went all the way back down.

This was changed to add more of a challenge.

Party Boy was a very late inclusion, being added a mere few weeks before beta testing began. I think it shows just how late it was when you play Nightmare.

He was originally just an easter egg, which you can still encounter in the main game.
