Herman got mutated by Katherine in a rampage while he was holding Maggie (his hare pet) and fell into the mutagen tank because Kat pushed him. The reason of the situation was that Selina shared all the personal information of Herman to Shredder and ordered the Foot Clan to kill him (basically Selina sold his only friend for her to keep living).
Katherine's mutation got triggered later, after the events of Herman's lost, the turtles (especially Raph) got mad at Kat (when she was out of control, that's a little thing she has I ain't gonna spoil). Kat and Raph had an argument and ended up Raph kicking her out of the lair and saying very bad things to her due to rage outburst. When she left, she got kidnapped by Shredder himself (I ain't gonna spoil the scene but it's epic).

This TMNT 2012 AU (Still with no name) is a rewrite of the original series but adding new characters and modifying interactions and episodes. Seasons are longer, new mutants, different ships (more or less) and surely it contains mature content (such as strong language, gore, suggestive themes and triggering scenarios).
