Ayy, one of the spriters for Undertoad here! While I can't speak for the creators of Utoad, I do still, at least, have some behind the scenes knowledge of the AU.
Just wanted to say that while I'm glad you do acknowledge the fact that so many people that are "fans" of UToad misinterpret the AU heavily, I fear this game may also fall into that same category. It's probably too quick to judge, but the fact that this is a "Last Breath" take on Undertoad is what worries me. A "Undertoad: Last Breath" take would assume Mario being in the "Sans role" and the existence of the concept of "roles", which is not the case. I know the previous iteration of UToad didn't entirely do itself justice with differentiating from UT, but I just wanted to give you a heads up on that.
With that being said, I suppose it's also not a huge problem either. I just, personally, don't like people labeling a project based on an AU and then being a gross misrepresentation of it. Makes sense? I do have some recommendations! Why not make a UToad fangame based on other characters, besides the "Sans role" or "Papyrus role"? This would help shine light on other aspects of UToad that are greatly underappreciated! Another recommendation would be making your own take on a "Mario X Undertale" AU and breaking your creative limits beyond UToad! This way, you have full control of what you want to do for your Mario Undertale AU ^^. Who could pass that up, right?
But again, not a huge deal. You could ignore me entirely and I wouldn't mind, but i just wanted to give you a heads up.

Undertoad : Justice Shall Prevail is modern day take on Undertoad Last Breath, using bits and pieces from preboot and reboot Undertoad, combined with some head canons to fill in the gaps in the story and plot!
*Long ago, an alien force invaded the lands, forcing the MUSHROOM and KOOPA kingdoms to align together.
* They attempted to resist attack, but even together, their armies faltered.
* Unable to defend themselves, the survivors fled to Mt. Corna.
* But even 5 Years after the force field was erected...
* Trouble seemed to follow them underground.
* An abandoned Toad fell into the mountain, who many confused for a Shroob.
* It was almost like they were one in the way they moved, the way they acted.
* And by their actions, the greatest the kingdoms knew, fell.
* But once seemed to not be enough, as all was set back to the start.
* Yet again, those who previously escaped the Shroobs, were struck down.
* During one of many of these repeated massacres...
* A previous hero came upon a place he had never seen before.
* A voice warned the plumber of a great danger coming ahead.
* They warned that everything this hero cared about would come crashing down.
* He was skeptical, but before he could find the will to question,
* Clouds began to form, giving him a glimpse into the foul future.
* Kammy, Luigi, Daisy... He watched as Friends, Foes, and Family alike were struck down.
* The hero's expression changed, he could deny these claims no longer.
* He could not sit idly by and allow this prediction to become reality...
* Now knowing the full extent of these injustices, he vows that he’ll make sure that only JUSTICE SHALL PREVAIL.
