Alright, I played it, and now for some thoughts.
The first time I played it, the game crashed because there wasn't anyone to be targeted. I think you should remove Beltboy's permanent Untargetable status effect and give it to him in the battle instead so that it can be removed once his transformation begins. This almost happened again when everyone was downed, so maybe it's the fault of RPG Maker (or me lol). If it still doesn't resolve anything, then maybe give some sort of signal to the player to not attack.
Another thing is that I agree with the people here, phase 4 should be nerfed, at least when Heart transforms into his injured form. (300 was the perfect amount of ATK for me, MAT can stay the same). Also maybe remove Regeneration from Heart the first time Beltboy transforms because it slows down the battle significantly.
I guess I should say some positive things. I think the fact that you're forced to lose until you become a mutant is great, really feels like Hart is desperately trying to stop them despite being outnumbered. The new animations, as I've said before, are a lot smoother and better (though it seems like Long Slam was fine for you). And of course, I appreciate the existence of this game in general because you made it from my own game.
That's all I can think of for now. Some minor gripes would be that Reginald disappears in the beginning cutscene, though even I couldn't find out the cause for this. And that I don't exactly understand what the ending was about, people from the LISA The Hopeful Discord server said that the characters felt...well, out of character. I don't really have a say in that though since I don't exactly understand the characters myself.
Really, really, REALLY good mod. There's a lot to appreciate here.
I don't think phase 4 being BS is necessarily a bad thing, either. It strikes me more of a "you're meant to die here" moment, so I fail to see it as a frustrating game over and more as just another ending to the fight. Plus, i love the "Benny Oberwinch is unstoppable! Unbeatable!" line too much to be mad at that healing move.
If I had a complaint it'd be that some of the heart mutant animations strike me as a little janky, but... it's a very minor complaint compared to what this has to offer. Props.
Also, that ending if you DO win... chills.

Played Hart's Perspective? Enjoyed taking control of a brutal rapist with a thousand men under his wing? Wanted him to transform into an aberration of what little humanity he has remaining?
