So, right after getting the injury on his cheek (from someone you’ll get to know very soon) he has a breakdown. The original drawing elaborated more, but I went with the less risqué counterpart (this drawing) the blood splatter is because, someone he cared about dearly died in front of him, and the blood splattered all over him, specifically his face.
