The plot of the game is starts with you (SANS) starting in your room (this is after the events of the genocide route) somehow in the reality you live in you are the only creature left in the underground (OR ARE YOU) because of Charas power now she has been able to skip to a new dimension so you and an unlikely character must save all creatures .
