Right side eye shaped
Plans:
Scream V mask shape
White-ish gray coloration, with dusty looking parts
Red spiral around right side eye
The red on the spiral will be slightly dripping, as if it was newly painted with slight imperfections
A jigsaw puzzle piece engraved somewhere around the cheek of the mask
Cracked a bit here and there, not as much as the Scream VI mask
Scratches on the paint here and there as well

The words "I Wanna Play A Game" at the bottom of the paper in blood red
The robes will be be like John Cramer's, being black with red accents on the edges
