Essentially, it is a point system in that the choices collectively contribute to the reveal of a hidden scene.
So throughout the full game's story, it will work in one of two ways:
multiple choices appearing in rapid succession within the same scene(pictured above) - context clues can help you determine which choice options are "related" in this manner
same choice in different scenes throughout story (but with different context each time)
Two things:
None of the scenes hidden by this mechanic will be necessary to complete the story or fulfill the achievements, they are just extra content for the people who want to really get into the project's lore
No hints will be given as to where these choices are, I assume any players who want to find them can do so on their own
